Students – If you can ANSWER these questions YES,
you may waive enrollment in the university insurance plan.
1. Will you be insured for the entire period for which you
are registering?
2. Does your plan provide inpatient and outpatient mental
health benefits?
3. Does your plan offer Prescription Drug Coverage?
4. For female students, does your plan provide maternity
benefits as any other temporary medical condition?
5. Does your plan have a provider network in the Tallahassee (Leon County)
area? Coverage must be available for urgent care and physician visits as well
as emergency
and local hospital care.
6. Does your plan offer a minimum of $100,000 in benefits yearly?
Have the following information available to complete the insurance
waiver:
1. The name of the person who pays your insurance premium
and their relationship to you.
2. The name, mailing address and telephone number of your insurance carrier.
3. Your policy number and group number. The group number is optional.
4. The effective date of your insurance policy. The effective date must be
at least one day before the beginning of the term for which you plan to register.
Do not enter a termination date for your policy if the plan automatically
renews on the anniversary of the policy start date.

IF YOU ARE PLANNING
TO PARTICIPATE IN INTERCOLLEGIATE ATHLETICS. The school sponsored
insurance will take care of your health care needs outside
of participation in intercollegiate athletics. The school
sponsored insurance will not cover your participation in
intercollegiate athletics.
